- ItemA randomized open-label clinical trial to evaluate convalescent plasma compared to conventional standard care management for SARS-CoV-2 (COVID-19) in hospitalized patients in the Dominican Republic(2022) Ángel Díaz BeltréAims: The potential benefit of blood therapy in the pandemic is an opportunity for breakthrough therapy. Still, it is also a test for countries to understand their blood supply system and its regulatory infrastructure to succeed in a potentially life-saving alternative for emergent pandemics. Methods: A multicenter, randomized, zero-phase exploratory study is to compare the efficacy and safety of Convalescent Plasma (CP, test group) with standard care therapy (control group) in hospitalized patients for SARS-CoV-2 (COVID-19). Results: Thirty-five patients were randomized; 18 (51.4%) corresponded to the treated group. There was a clinical improvement over time for both groups (p < 0.001), but CP didn’t show a significantly different result than standard treatment for non-critically ill patients infected with COVID-19 in the Dominican Republic (p = 0.058). The appearance of dyspnea, the increase of >50% of pulmonary infiltrates between 24 and 48 hours of evolution, and positive qualitative polymerase chain reaction (PCR) results improved significantly in the control group. Therefore, convalescent therapy presented a significant recovery in these signs and symptoms. No adverse events were reported. Conclusion: This prospective study serves as a pilot to propose an investigation with a representative sample to evaluate the benefits of this therapy’s effects in a bigger population. The most significant advantage of Convalescent Plasma (CP) therapy was obtained in the first six days, where the improvement in clinical categorization was faster, suggesting CP is best for early and mild cases of COVID-19. Actions for enrolling volunteers for this study were not productive, an opportunity for the Dominican Republic health authorities to improve their blood donation strategies and support blood availability

Prismas y estimaciones(2023) María Virtudes Núñez Fidalgo; Edith RamírezEste libro presenta una serie de propuestas de investigación, intervención y mediación sobre las alteraciones del lenguaje y la comunicación en contextos clínicos. El leitmotiv de la obra es el sujeto afectado por una alteración del lenguaje: su imagen social, su idiosincrasia cultural y lingüística. La metodología incluye tanto aportaciones de corte experimental como basadas en corpus, o reflexiones metodológicas y epistemológicas, con un enfoque pluridisciplinar y ampliamente pragmático donde la sociología, las ciencias cognitivas, la neurología, las ciencias de la educación y la reflexión filosófica complementan a la lingüística. Refleja, en suma, un clima intelectual y académico, un "estado del arte": el de la lingüística clínica, en general, y en el ámbito hispánico en particular.
- ItemAfrontamiento emocional y formación de postgrado en tiempos de Covid-19,(2021) Ysabel Noemí Tejeda; Olga Jape Julio Adrián RivasSe realizó una investigación descriptiva, prospectiva y de corte transversal, cuyo propósito fue conocer las diferentes dinámicas establecidas por participantes del programa de Maestría en Alimentación y Nutrición (MAN) en cuanto al afrontamiento emocional ante la pandemia por COVID-19 y en la continuidad de la formación de postgrado. Para tales fines, se aplicó un cuestionario en línea a los 33 maestrantes del Programa, con análisis cuantitativo descriptivo e inferencial y análisis cualitativo consistente en descripciones detalladas de situaciones observables que incorporan las experiencias, actitudes y reflexiones de los participantes. Los resultados relevantes muestran que el 85% de los participantes no habían sido diagnosticados positivos al COVID, y ninguno de los participantes de la Maestría señaló relación directa con la prestación de servicio en la primera línea de atención a COVID-19. El 97% usaban WhatsApp, seguidos por Facebook con 76% y 9% Skype; respecto a Moodle como herramienta de gestión de aprendizaje, 12% tenía experiencia, y planteada Microsoft Teams como alternativa para el aprendizaje a distancia, el 21% manifestó experiencia. Aun con estas frecuencias porcentuales, los maestrantes IDI-UNICyT 2021 837 Actas del VI Congreso Investigación, Desarrollo e Innovación permanecieron apegados a metas personales. Se hallaron emociones experimentadas con mayor frecuencia como agotamiento físico y mental, ansiedad, fatiga, angustia anticipatoria y tristeza. Ante la amenaza con la tasa de transmisión y letalidad se asume acertada los resultados respecto a las emociones manifestadas por los sujetos informantes. Se evidenció que residir en zona urbana influyó positivamente, dada la relación con la alta calidad en esta zona en los servicios de telecomunicaciones.
- ItemAn integrated urban gardens and peer nutritional counseling intervention to address food insecurity among people with HIV in the Dominican Republic(2021-05) Amarilis Then PaulinoFood insecurity negatively affects HIV prevention and care, and sustainable interventions are needed. Here we describe the development of an integrated urban gardens and peer nutritional counseling intervention to address food insecurity and nutrition among people with HIV, which included: (1) peer nutritional counseling, (2) gardening training, and (3) garden-based nutrition and cooking workshops. The intervention was developed using community-based participatory research over multiple years and stages of data gathering and implementation and evaluation. Lessons learned include the importance of cross-sectoral partnerships to achieve multifaceted, integrated, and sustainable interventions and a shared commitment among partners to an ongoing cycle of action-oriented research, and the need for home-based and community-based gardens to enhance food security and social support. The development process successfully combined an evidence-based framework and community engagement to yield a multicomponent yet integrated food security and nutrition intervention appropriate for people with HIV and potentially adaptable for other chronic conditions.

El mesocarpio del maracuyá puede ser aprovechado por los macronutrientes y micronutrientes que contiene, por sus pigmentos, y por el contenido de varios nutrientes con propiedades terapéuticas.

El objetivo del presente artículo es presentar una reflexión sobre la bioética y acciones de salud pública. Es un artículo de revisión que incluye autores con diferentes posturas. La bioética a pesar de su aparente enfoque individual es vital para cumplimiento de las funciones esenciales en salud pública, garantía del derecho a la salud y respecto a la dignidad humana.
- ItemChanges in HIV internalized and experienced stigmas and social support after an urban gardens and peer nutritional counseling intervention among people with HIV and food insecurity in the Dominican Republic(2024-09-01) Amarilis Then"Background: Food insecurity and HIV-related stigma negatively affect HIV outcomes. Few studies have examined how food security interventions affect HIV-related stigma and social support. Setting: Two HIV clinics in the Dominican Republic. Methods: A pilot cluster randomized controlled trial of an urban gardens and peer nutritional counseling intervention was conducted to examine outcomes of HIV-related stigmas and social support. Adult patients (≥18 years of age) with moderate or severe household food insecurity and evidence of suboptimal ART adherence and/or a detectable viral load were enrolled; standard measures of internalized and experienced stigmas and social support were collected at baseline and at 6 and 12 months. Intervention clinic participants received training and materials from agronomists for a home garden, 3–4 sessions of nutritional counseling from the clinic's peer counselor, and a garden produce cooking workshop facilitated by professional nutritionists. Results: Of 109 study participants (46 intervention and 63 control), 103 (94%) completed 12-month follow-up. Difference-in-differences multivariate longitudinal linear regressions adjusting for sociodemographic factors found that intervention participants had reduced internalized stigma by 3.04 points (scale 0–32) at 12 months (P = 0.002); reduced probability of experiencing HIV-related stigma or discrimination in the past 6 months (20 percentage points at 6 months, P = 0.05 and 25 percentage points at 12 months, P = 0.02); and modestly improved social support at 12 months (1.85 points on 30-pt scale, P = 0.093). Conclusion: A fully powered, larger trial is needed to establish the efficacy of the intervention and assess pathways by which the intervention may improve HIV stigma and social support."
- ItemClinical manifestations of chikungunya among university professors and staff in Santo Domingo, the Dominican Republic(2017) Matilde Peguero"Objective. To further characterize chikungunya virus infection and its associated clinical manifestations, using a sample of university professors and staff in Santo Domingo, the Dominican Republic. Methods. A cross-sectional study with quota sampling by department was performed to obtain a convenience sample of professors (n = 736) and staff (n = 499) at the Universidad Autónoma de Santo Domingo. Surveys were used to collect demographic and infection data during the fall term of 2014. Univariate and bivariate analyses were carried out to quantify infection and clinical manifestation prevalence and to assess relationships of these outcomes with age, sex, and acute phase duration. Results. Of 1 236 participants, 49% reported infection (professors = 41%; staff = 61%). Of these, 53% also reported the presence of chronic effects, largely arthralgia (48%). Significant relationships were observed between reported infection and sex (P = 0.023), age (P < 0.001), and occupation (P < 0.001). More headache (P = 0.008) and edema (P < 0.001) in females, more headache (P = 0.005) in younger subjects, and more myalgia (P = 0.006) in those with longer acute symptoms were found. Additionally, more chronic arthralgia (P < 0.001; P = 0.003) and chronic edema (P < 0.001; P = 0.001) in females and older subjects, and more chronic myalgia (P = 0.041) and chronic edema (P = 0.037) in those with longer acute symptoms were observed. Conclusions. To the authors knowledge, this is the first population-based chikungunya prevalence study in the Dominican Republic, and the first to explore clinical manifestations in a university setting. The findings reflect results from studies following the 2005 – 2006 Reunion Island outbreak: prevalence of infection and chronic arthralgia, as well as associations with sex, age, and acute intensity. Longitudinal research can provide further insight into these effects "
- ItemCOVID-19 pandemic and health worker stress: the mediating effect of emotional regulation,(2020) Vahid Nouri KandanyBackground/Introduction Psychological and physical well-being of health personnel has been significantly affected by COVID-19. Work overload and continuous exposure to positive COVID-19 cases have caused them fatigue, stress, anxiety, insomnia and other detriments. This research aims: 1) to analyze whether the use of cognitive reevaluation and emotional suppression strategies decreases and increases, respectively, stress levels of health personnel; 2) to quantify the impact of contact with patients with COVID-19 on stress’s level of medical staff. Method Emotion regulation strategies and stress level were evaluated in 155 Dominican physicians by means of psychological tests with adequate levels of reliability. In addition, a questionnaire created by the researchers quantified the impact that contact with those infected had on their stress levels. Results Contact with patients with COVID-19 predicts increased use of emotion suppression strategies, although is not associated with the use of cognitive reevaluation. These findings lead to an even greater increase in stress on health care providers. Conclusions Contextual contingencies demand immediate responses and may not allow health personnel to use cognitive re-evaluation strategies, leaning more towards emotion suppression. However, findings regarding high levels of stress require the implementation of intervention programs focused on the promotion of more functional emotion regulation strategies. Such programs may reduce current stress and prevent post-traumatic symptoms.

- ItemDiplomado de investigación con enfoque en la salud pública sistematización experiencia de formación en investigación en salud(2021-05-17) Adelaida OresteEl diplomado en investigación con enfoque en la Salud Publica/Salud Colectiva, fue realizado con el propósito de reducir las brechas en el nivel de conocimiento sobre investigación científica, en docentes universitarios, gestores y prestadores de servicios de salud en la Republica Dominicana. Se asumió el enfoque de la Salud Publica por la necesidad de reorientar supuestos teóricos y metodológicos del enfoque biologista y hospitalocéntrico, que predomina en la formación de recursos humanos en salud, redirigiendo la formación hacia una visión integral y crítica sobre los problemas de salud. Acá se entendió a la salud como proceso histórico socialmente determinado. Este cambio de paradigma requirió el uso de metodología congruente con los valores y principios del enfoque asumido, la metodología de investigación acción participativa, creando un proceso de enseñanza-aprendizaje dialógico y colaborativo en el cual el análisis y la reflexión propositiva, dieron paso a una mirada y acción más integral sobre los problemas de salud. Fue ejecutado en un periodo de siete meses (7) con seis (6) módulos desarrollados a través de distintas estrategias y técnicas didácticas. Los temas fueron Salud Publica/Salud Colectiva, Investigación Científica en Educación Superior, Investigación Cualitativa en Salud y el Proceso de Investigación Científica, todos transversalizados por tres ejes: El derecho universal a la salud, Determinación Social, Equidad en salud y de género. Se formó un comité de dirección, cinco (5) grupos de trabajos, un equipo de coordinadores, un equipo de docentes y conferencistas nacionales e internacionales, entre los que hubo un ex premio Nobel de la Paz. Entre los resultados destacan alto porcentaje de permanencia, 48 personas iniciaron y 41 egresaron, para un 85.4 %, 19 de estos realizaron una pasantía en la Universidad de Guadalajara, Jalisco y en la Universidad Nacional Autónoma de México y se elaboraron cinco (5) propuesta de investigación, de los cuales dos (2) han sido ejecutadas.
- ItemEducación superior e innovación: modalidad híbrida, una mirada ecosistémica(2023) Olga Jape CollinsUn modelo innovador debe generar un espacio de discusión y reflexión de los docentes sobre los avances de los jóvenes y de las metodologías aplicadas con miras a dar la flexibilidad y adaptabilidad del modelo, porque innovar es pretender generar cambios sustanciales que propicien mejoras permanentes en la educación, pero, también debe permitir la identificación delas herramientas adecuadas para detectar los estilos de aprendizaje en las instituciones de educación superior. Por ello, un grupo de docentes de varias universidades se plantearon un ecosistema con el objetivo de identificar las estrategias, métodos y entornos que permitieran un ambiente favorable a la adquisición de un aprendizaje significativo obteniéndose catorce buenasprácticas que pueden ser aplicables en otras universidades para mejorar el proceso de enseñanza-aprendizaje. La consideración de las iniciativas estudiantiles en el aprendizaje resultórelevante por su objetividad y su inclusión auténtica, además de que establece un sentido de pertenencia y corresponsabilidad de estos en el sistema educativo. - ItemEfecto del programa de transferencia monetaria condicionada en la incidencia de tuberculosis pulmonar en República Dominicana (2012-2018)(2024-09-18) Francisco Martín DuarteIntroduction: Pulmonary tuberculosis is a public health problem in several countries in Latin America and the world, due to its high incidence and social impact. The objective of this study was to evaluate the impact of the CCT program on the incidence of pulmonary tuberculosis (PTB) in Health Region III of the Dominican Republic during 2012-2018. Method: An observational study with a mixed ecological design was carried out. New cases of TBP and social programs were analyzed. The coverage of social programs was the main variable, and the TBP incidence rate was the dependent variable. Other covariates were considered. Results: The results showed that the TBP incidence rate did not decrease in Health Region III despite an increase in the coverage of CCT social programs and a decrease in the poverty level. The CCT "Eating First" program had a significant impact on reducing poverty and other social determinants in Health Region III. Conclusions: Our findings support previous research suggesting that CCT programs are effective in reducing poverty and addressing some social determinants of health. However, they do not support the use of CCTs as an isolated measure to reduce new cases of TBP, since poverty must be addressed in a comprehensive, holistic and inclusive manner to achieve a more significant impact on diseases with high prevalence and endemicity
- ItemEffect of cervical lesions on fracture resistance and failure mode of maxillary central incisors restored with fiber posts and complete crowns(2022-05-04) Martín Domingo Santos Pantaleón"Purpose To investigate the effect of a cervical cavity extending 1 mm apical to the cemento–enamel junction (CEJ) on fracture resistance and failure mode of maxillary central incisors that have been treated endodontically, present with complete and incomplete ferrules, and are restored with and without a fiber post. Methods and Materials 50 intact human maxillary central incisors were divided into five groups (n=10): CG (control group) 6-mm fer-rule height, no cervical cavity, and without post; (CO) 6-mm ferrule height without post, with a cervical cavity (access to root canal and cervical cavity restored with composite resin), cervical cavity; and post with ferrule heights of 1 mm (CP1), 2 mm (CP2), and 6 mm (CP6) restored with fiberglass post and composite resin core. After complete metal crowns were cemented on all specimens, they were subjected to thermal cycling (6000 cycles, 5°C/55°C), followed by immediate testing of fracture resistance. After failure, the specimens were sectioned buccolingually to evaluate and identify the mode of failure. The data were analyzed with an analysis of variance (ANOVA) and the Student–Newman–Keuls multiple comparison tests (α =0.05). Results A 1-mm ferrule height (CP1) fracture resistance was significantly lower (531±125 N) compared to the 6-mm ferrule height (CP6) (769±175 N) (p<0.05). With respect to the groups with similar residual dentin, with and without a cervical cavity, CG (667±119 N) and CO (668±119 N), the presence of a post (CP6) increased the resistance to fracture, although no statistically significant difference was demonstrated. Partial decementation was observed in all specimens of CG and CP6, in nine of CP1 and CP2, and in three in CO. Root fractures occurred in 23 specimens. The root surface was exposed 2 mm below the CEJ to simulate bone level. Propagation of subosseous cracks occurred in four specimens in CG and CP2, in seven specimens in CP6, in two specimens in CP1, and in six specimens in CO. All were considered catastrophic failures. Conclusions Within the limitations of this study it is suggested that, when restoring an endodontically treated maxillary central incisor that has a cervical lesion and needs to be restored with a complete crown, a fiber post is cemented to improve fracture resistance."

- ItemEffects of Endodontic Infections on the Maxillary Sinus: A Case Series of Treatment Outcome(2021-07) Fatima BuenoThis article shows the follow-up of several cases of maxillary sinusitis of dental (usually endodontic) origin, with different manifestations, diagnostic challenges, and outcomes.Cases from 14 patients from 3 countries and treated by 7 different endodontists are presented, all of them with inflammatory sinus changes represented by mucositis, osteoperiostitis, and/or partial/full obstruction. All cases showed dental and/or sinus signs/symptoms that resolved after dental management. In 13 cases, the sinus condition had an endodontic origin, 4 of them concurrently with periodontal involvement. In 1 case, sinusitis was caused by trauma to the face. All cases but 1 had a satisfactory response of the periradicular tissues and maxillary sinus to treatment that consisted of root canal therapy, root amputation, extraction, or trauma management.The successful management of most cases reported in this article emphasizes the importance of endodontics as a specialty engaged in saving teeth and promoting health not only in the oral cavity but also in other areas that may be affected by infections of endodontic origin, including the maxillary sinus.
- ItemEl enfoque curricular centrado en competencias: sus beneficios en educación en ciencias de la salud(2023-09-06) Olga Jape CollinsSe presenta una revisión bibliográfica determinando beneficios fundamentales de la implementación del enfoque basado en competencias en la educación en Ciencias de la Salud. El objetivo primordial fue analizar cómo este enfoque fomenta habilidades interpersonales, comunicación efectiva, adaptabilidad, integración teoría-práctica y evaluación precisa del rendimiento estudiantil en este campo crucial. El método de revisión se basó en una búsqueda exhaustiva y un análisis crítico de investigaciones, artículos y libros relacionados con la educación en salud y el desarrollo de competencias en este contexto, para respaldar y enriquecer las afirmaciones realizadas. Los resultados de esta revisión en progreso demuestran que la perspectiva basada en competencias fortalece significativamente la formación de profesionales altamente competentes en Ciencias de la Salud, preparándolos para enfrentar los desafíos complejos de la atención en salud contemporánea. La relevancia de este enfoque curricular metodología radica en su capacidad para alinear la educación con las demandas de un mundo cambiante. Además, la integración de la evaluación precisa del rendimiento estudiantil en este enfoque garantiza que los egresados estén realmente preparados para ejercer en un entorno profesional exigente y para contribuir de manera significativa a la mejora de la atención clínica individual y la salud colectiva. En conclusión, la inclusión del enfoque basado en competencias en la educación en Ciencias de la Salud proporciona a los estudiantes una sólida base para abordar las diversas situaciones en el entorno de la salud y les dota de habilidades esenciales para trabajar en equipos interdisciplinarios y comunicarse eficientemente con pacientes y colegas.

- ItemESCAPE pain trial –The effects of Curcumin in pain relief in women diagnosed with primary dysmenorrhea: A triple-blinded, placebo-controlled, phase II, randomized clinical trial protocol(2020-08-12) Vahid Nouri KandanyPrimary dysmenorrhea affects many women, being a major cause of absenteeism and reduced productivity at work and at school. Although non-steroidal anti-inflammatory drugs (NSAIDs) are a good treatment option, up to 18% of women show no response or present allergic reactions and adverse events. Curcumin has antispasmodic, antinociceptive, and both specific and nonspecific anti-inflammatory effects, with good tolerability and safety. To date, no previous trial involving curcumin and dysmenorrhea pain has been performed. Therefore, our main goal is to assess the efficacy of curcumin for pain relief among women with primary dysmenorrhea, along with determining curcumin’s adverse effects and tolerability profile.
- ItemEvaluation of the short-term response in refractory trigeminal neuralgia treated with CyberKnife(2023-11-06) Maricely Ámbar Pérez FernándezObjectives. Stereotactic radiosurgery combined with pharmacological treatment is currently one of the most acceptable alternatives for the treatment of trigeminal neuralgia (TN). Our primary endpoint was to report the short-term response (one month) outcomes of 10 patients with refractory TN after CyberKnife® (CK) radiosurgery; secondary endpoints were to assess early side effects and complications. Methods. Ten female patients with TN refractory to pharmacological and/or nonpharmacological treatment received a single dose of 90 Gy with CyberKnife® radiosurgery. Clinical and demographic characteristics were obtained from medical records. The visual analog scale (VAS) was used to assess facial pain before as well as seven, 15, and 30 days after treatment. Friedman test was performed to evaluate pain relief in treated patients. Results. All patients responded well to the CK and experienced initial adequate pain relief during the first 30 days (p<0.001). No significant differences (p=0.661) were found between six patients who received CK radiosurgery as the first treatment option and those who underwent other non-pharmacological treatments. One patient was re-irradiated with 75 Gy. Transient facial paresthesia was observed in 3/10 patients without any other complications. Conclusion. High-tech CK treatment is safe, non-invasive, fast, with minimal side effects, and effective in achieving short-term pain relief in patients with refractory TN, even in those with multiple previous interventions. Given these results, we recommend evaluating CK radiosurgery as the first-line treatment of choice for trigeminal neuralgia refractory to pharmacological treatment.
